  1. DISA valve flap/pivot worn or O-ring failed ($80-200)
  2. DISA adjuster motor failed
  3. Broken DISA flap (can drop debris into the engine - inspect)

Check first: On M54/N52 inspect the DISA O-ring and that the flap is intact and not loose — a broken flap is a serious ingestion risk.
